چکیده

Objective: To describe the range of pathology causing pleural eVusions in HIV infected patients with acute respiratory episodes and to attempt to identify whether any associated radiological abnormalities enabled aetiological discrimination. Methods: Prospective study of chest radiographs of 58 consecutive HIV infected patients with pleural eVusion and their microbiological, cytological, and histopathological diagnoses. Results: A specific diagnosis was made in all cases. Diagnoses were Kaposi’s sarcoma, 19 patients; parapneumonic eVusion, 16 patients; tuberculosis, eight patients; Pneumocystis carinii pneumonia, six patients; lymphoma, four patients; pulmonary embolus, two patients; and heart failure, aspergillus/leishmaniasis, and Cryptococcus neoformans, one case each. Most eVusions (50/ 58) were small. Bilateral eVusions were commoner in Kaposi’s sarcoma (12/19) and lymphoma (3/4) than in parapneumonic eVusion (3/16). Concomitant interstitial parenchymal shadowing did not aid discrimination. A combination of bilateral eVusions, focal air space consolidation, intrapulmonary nodules, and/or hilar lymphadenopathy suggests Kaposi’s sarcoma. Unilateral eVusion with focal air space consolidation suggests parapneumonic eVusion if intrapulmonary nodules are absent: if miliary nodules and/or mediastinal lymphadenopathy are detected, this suggests tuberculosis. Conclusions: A wide variety of infectious and malignant conditions cause pleural eVusions in HIV infected patients, the most common cause in this group was Kaposi’s sarcoma. The presence of additional radiological abnormalities such as focal air space consolidation, intrapulmonary nodules, and mediastinal lymphadenopathy aids aetiological discrimination. Vascular endothelial growth factor (VEGF) in inflammatory and malignant pleural eVusions

Background—Investigation and management of pleural eVusions is an important clinical problem yet the pathogenesis of pleural fluid accumulation is poorly understood. Vascular endothelial growth factor (VEGF) is a potent inducer of capillary permeability that is produced by both malignant and inflammatory cells. A study was undertaken to determine whether VEGF has a potential pathogenic role in ...

Short reports Absence of HHV-8 DNA sequences in malignant mesothelioma

Human herpesvirus 8 (HHV-8) associated primary eVusion lymphomas arise and grow in the body cavities as eVusions, but it is not known whether the lining of body cavities and mesothelium derived malignancies are potential targets of HHV-8 infection. We examined a series of 13 diVuse malignant mesotheliomas and four mesothelial cell rich eVusion samples of non-neoplastic aetiology from nonimmunod...

Tuberculous pleurisy is more common in AIDS than in non-AIDS patients with tuberculosis.

OBJECTIVE To investigate the incidence, clinical features, and treatment of tuberculous pleurisy in AIDS patients. METHODS We reviewed all cases of pleural tuberculosis in AIDS patients in South Carolina from 1988 through 1994. Clinical findings, test results, treatment, and outcome were analyzed. MAIN RESULTS Twenty-two (11%) of the 202 AIDS patients with tuberculosis had pleural involveme...

The Role of Thoracoscopy in the Diagnosis of Pleural Effusion of Unknown Origin

BACKGROUND AND OBJECTIVE: Video-assisted thoracoscopic surgery is preferred over conventional methods, such as pleural biopsy, in the diagnosis and treatment of patients with pleural effusion. Considering that Imam Khomeini Hospital is facing a large volume of patients with pleural effusion of unknown origin referred from other centers, the present study was conducted to investigate the role of...

Transforming growth factor â2 (TGFâ2) produces eVective pleurodesis in sheep with no systemic complications

Background—We have recently shown that transforming growth factor (TGF)â2 induces eVective pleurodesis in rabbits. However, rabbits have a thin pleura while humans have a thick visceral pleura. The eVect of intrapleural administration of TGFâ2 in animals with a thick pleura and its associated systemic eVects have not been investigated. This study was undertaken (1) to develop a new animal model...
